We have provided wholesome healthcare in Fort Worth since 1990. Dr. Damico graduated in 1987 with a Medical degree from The University of Texas Medical School at Houston. He is board certified with the American Board of Family Practice.

Please click on the link titled "FORMS" to complete all paperwork listed and bring them with you to your appointment. Please be sure to bring all medications you are currently taking to your appointment, as well as your Medicare card and identification.
For your convenience we accept cash, check, and major credit cards: Visa, Mastercard, Discover and American Express. These are our only forms of accepted payment. Thank you for understanding.
For existing patients, only private pay (cash, etc. listed above) and Medicare are accepted. Managed Medicare plans are NOT accepted and the patient will be responsible for the full charges of services rendered. If you have received a call from "Medicare" claiming that they can help you save on your premiums and subsequently you switched your "Medicare" coverage, you may be on a managed Medicare plan. This is not the same as Medicare and we do not have any contracts with any managed Medicare plans (eg. "Medicare" coverage through other commercial insurance companies). Please verify this information with your primary insurance carrier before your next visit.
Medicare patients are seen Non-Assigned. This means you are required to pay the limited charge Medicare assigns for services directly to our office at the time of service. We will file your Medicare (Non-Assigned) and any payment will be sent directly to you from Medicare.
Existing and current patients: All medications are filled at the time of your office visit. Please call 817-738-9268 after hours should you need to reach Dr. Damico in the case of an emergency. You may also call 911 for emergency services.
